بررسي رابطه بين عدالت توزيعي و سلامت در ايران: 1385-1363

چکيده

مفهوم مدرن عدالت توزيعي بر توزيع عادلانه منابع و منافع جامعه بين اقشار مختلف تأکيد دارد. از طرف ديگر سلامت بعنوان يکي ديگر از مقوله هاي رفاه اجتماعي تحت تأثير وضعيت شاخص هاي عدالت توزيعي قرار مي گيرد. در اين پژوهش رابطه بين شاخص هاي عدالت توزيعي و سلامت طي سال هاي 1385-1363 مورد بررسي قرار گرفته است. اين مطالعه از نوع تحليل همبستگي اکولوژيکي مي باشد. براي اندازه گيري عدالت توزيعي بعنوان متغير مستقل از شاخص هاي نابرابري درآمدي، فقر و مخارج دولت در سلامت استفاده شده است و براي بررسي سلامت از شاخص هاي اميد به زندگي و مرگ و مير کودکان زير 5 سال استفاده شده است. براي اجراي پژوهش ابتدا دادهاي مورد نياز مطالعه را ازمرکز آمار تهيه کرده و سپس با استفاده از نرم افزار stata و excel شاخص هاي مورد نظر استخراج شدند. و در نهايت روابط بين شاخص ها توسط نرم افزار spss مورد تحليل قرار گرفت.

بطور کلي طي سال هاي مورد بررسي روند شاخص هاي نابرابري درآمدي و فقر نزولي و روند شاخص هاي سلامت صعودي مي باشد. تحليل رگرسيون نشان داده که شاخص هاي عدالت توزيعي با شاخص هاي سلامت رابطه دارند. اما با انجام تحليل همبستگي جزئي رابطه بين شاخص ها معني دار نخواهد بود. نتايج رگرسيون دو متغيره نشان مي دهد که فقر نسبت به نابرابري درآمدي قدرت تبيين کنندگي بيشتري دارد و انجام رگرسيون سه متغيره مشخص شد مخارج دولت در سلامت بيشترين قدرت تبيين کنندگي شاخص هاي سلامت را دارد. با توجه به نتايج طي سال هاي مورد بررسي، با بهبود شاخص هاي عدالت توزيعي شاخص هاي سلامت نيز ارتقاء يافته است. همچنين در ميان شاخص هاي عدالت توزيعي مورد بررسي مخارج دولت در سلامت بيشترين رابطه و قدرت تبيين کنندگي را نشان مي دهد. لذا به نظر مي رسد توجه دولت به سرمايه گذاري در بخش سلامت جهت نيل به اهداف عدالت توزيعي و ارتقاء شاخص هاي سلامت مطلوب مي باشد.

Abstract

Introduction: modern concept of distributive justice places importance on equitable distribution of resources and advantages in society. Also, health as one of social welfare sub-category is under affects of distributive justice indicators. In this study, the relationship between health and distributive justice indicators, from 1362 to 1385, has been studied.

Methods and materials: this study is an ecological analysis. For measuring distributive justice as independent variable, income inequality, poverty and government expenditures indicators, and for measuring health, life expectancy and infant mortality rates were used.  At the first, needed data gathered from national statistic center and then using STATA AND excel soft wares, needed indicators derived and finally associations between indicators with using SPSS software got analyzed.

Findings: in all, at the studied years poverty and income inequality indicators were declining and health indicators were escalating. Correlation analysis indicated that there is relationship between distributive justice indicators and health indicators but with controlling government expenditures in partial correlation, there is no relationship between mentioned indicators. with using  bivariate regression ,poverty was more explanatory than income inequality .also with introducing government expenditures in regression ,this fact revealed that government expenditures in health has most explanatory among distributive justice indicators.

Conclusion: taking account results of study, distributive justice indicators have relationship with health indicators and in studied years with increasing distributive justice indicators, health indicators have improved. also, among the studied distributive justice indicators, government expenditures in health have the most relationship and explanatory power. So, it seems that government investment in health sector for obtaining distributive justice goal and promoting of health is necessary.
